As a courtesy, JA Physio offers direct billing to most insurance companies. Since every insurance plan is set up differently, we recommend for patients to check the following with their insurance provider:

  • If a Doctor's referral is required
  • If plan allows electronic billing
  • If plan allows the clinic to receive payment on your behalf

Information we will need in order to submit for you:

  • Insurance card or info (policy/contract number, Certificate/ID)
  • Doctor’s referral if required. (It is best to check with your insurance company if you are required to have one).
  • Policy holder’s name and date of birth if not self (spouses and children)
  • Policy holder's signed permission on the "assignment of benefit" form provided to you at the time of booking.

Note: that if both you and your spouse/partner have coverage: submission to your insurance has to take place first. For children: if both parents have extended health benefits, the parent whose birthday comes first in the year will be the one that is billed first.

Frequently Asked Questions

The term "electronic submission" means that the clinic is able to submit to your insurance plan on your behalf.

The term "assignment of benefits" means that the clinic receives the payment directly from the insurance company. If your plan does not allow assignment of benefits, this means that the payment will go directly to the plan member.

We are unable to choose where your payment will be directed to, please check with your insurance plan to see if your plan has assignment of benefits.

If both you are your partner have insurance. The primary plan member which is yourself must be billed for first. Whatever your plan does not cover, you will have to pay and submit yourself to your partners plan.

The parent whose birthday is first in the calendar year would be the parent whose plan gets billed first when billing through insurance.

If both partners have insurance, your plan must be billed to your insurance company first, anything not covered by your plan, we kindly ask that you pay the remainder and submit to your partners plan as we are unable to do coordination of benefits.
